What is the PICO framework?
PICO is a structured approach for developing focused research and clinical questions. It is frequently used in evidence-based healthcare because it separates a broad topic into concepts that can later inform a literature search.
P — Population
The patient group, population or health problem being studied.
I — Intervention
The treatment, programme, policy, exposure or approach being evaluated.
C — Comparison
The alternative intervention, usual care, control condition or other relevant comparator.
O — Outcome
The measurable result, change or effect that the research is interested in.
PICO question examples
Weight management
In adults living with obesity, how do structured weight-management programmes compared with usual care affect weight reduction?
Falls prevention
In older adults living in care homes, how do exercise-based falls-prevention programmes compared with usual care affect the incidence of falls?
Diabetes education
In adults with type 2 diabetes, how does structured diabetes education compared with standard information affect glycaemic control?
PICO question FAQs
What does PICO stand for?
PICO stands for Population or Patient, Intervention, Comparison and Outcome. It helps transform a broad topic into a structured research question.
Do I always need a comparison?
No. Depending on the research question, the comparison may be usual care, another intervention, no intervention or may not be required at all.
Can I use PICO to search databases?
Yes. PICO concepts can provide a starting point for identifying search terms. A strong database strategy normally expands each important concept with synonyms, spelling variants and relevant subject headings.
